I'm going to toss Riven, Shadow of the Colossus, and BioShock into the ring, less than because I actually think any of them meet all your criteria than because I think it's interesting to think about where they don't meet that standard. (And though you said not to mention Final Fantasy, FFVII definitely meets your cruelty standard.)
