There was an essay in the back of one of the latest Pluto releases by Viz written about Urasawa's relationship to Tezuka by a Japanese manga critic. IIRC she read Monster as a possible allegory for Tezuka's hatred for but continual resurrection of Astro Boy, which I'm pretty sure the essay says he called a "monster."

In any case, along similar lines and considering Urasawa looks up to and was heavily influenced by Tezuka, Monster could be read such that Johan represents Tezuka's cruelty, which Urasawa revives if only to append an optimism and humanism which follows the aftereffects of Tezuka's cruelty (Tenma).

Just for fun of course, I think it's probably a stretch but that's what makes it fun.
